Description	Entry No. 103 in the Register Book of Deaths No. 3A for the sub-district of Maidstone, Kent. Death certificate for Phillip Edwin Challis (Otherwise known as Philip Edwin Challice), who was a retired Sadler. He is recorded as passing on the 9th February 1959 at Oakwood Hospital, Maidstone, aged 86. The causes of death were given as ‘Myocardial Degeneration’ and ‘Senile Dementia’. The death certificate was registered on 11th February 1959.
Notes	Noted at top left of the document that ‘The statutory fee for this certificate is 3s. 9d. Where a search is necessary to find the entry, a search fee is payable in addition.’ Persons mentioned: • Gordon McLaren M.D. (Certified causes of death) • L.M. Rosier (of 15 Loxwood Avenue, Worthing) – Niece (Informant of Death) • C.H. Mardon (Registrar) • Alfred G. Chandler (Superintendent Registrar of District of Maidstone, Kent) Details all typed with typewriter except signature of A. G. Chandler (Superintendent Registrar of District of Maidstone, Kent). In the ‘Name and Surname’ section, the name ‘Phillip Edwin Challis’ has had a light box drawn around it in pencil, and ‘Challice’ of the second spelling given has been underlined twice. Perhaps indicates a previous attempt at someone tracing a family tree and noting variant spellings?
